Blepharoplasty (Cosmetic Lid Surgery)

The removal of excess tissue from the eyelid is called a blepharoplasty.
This procedure can be performed on the upper or lower eyelids, but may also involve the need for surgery on adjacent areas of the face, such as the forehead or cheeks.
This operation is considered to be cosmetic by most insurance companies unless a significant loss of side vision is present. In almost all cases, pre-authorization of surgery is needed. Unfortunately, such pre-authorization is impossible to obtain for Medicare patients.

Ptosis (Droopy Eyelid)

Although this is similar to excess tissue of the eyelid, "ptosis" of the lid is present when the muscle that raises the lid is affected by disease or trauma. Both ptosis and excess tissue may be present at the same time, with both conditions needing repair.
Ptosis can be congenital, meaning that the patient was born with the problem, or it can be acquired.
A number of conditions can cause ptosis, and it is important to diagnose the reason for the condition so that the most effective therapy can be recommended.

Thyroid Eye Disease

Thyroid disease can cause a number of thyroid-related eye problems.
These can include sight-threatening compression of the optic nerve, double vision, irritation and bulging of the eyes. One of the more challenging problems we see, this problem in particular demands close cooperation between the ophthalmic plastic surgeon, your endocrinologist, ophthalmologist and primary care physician.

Eyelid Malposition

A number of problems can cause malposition of the eyelids. An eyelid may turn inward, an "entropion", or it may turn outward, an "ectropion".
Many things can cause these problems including scar tissue or facial nerve palsy(Bell's Palsy).

Tearing

-
One of the most common complaints we see is tearing. Tearing can be caused by many, many underlying problems, in children and adults. Sometimes it can even be caused by a lack of lubricating tears, with an overproduction of watery tears. A thorough examination is needed to deal with this very frustrating problem.

Facial Spasm

"Facial Spasm" is usually caused by one of two problems
- Blepharospasm: this is part of a generalized movement disorder that affects both sides of the face, and may involve other facial or neck muscles as well.
- Hemifacial Spasm: this affects only one side of the face, and is usually caused by pressure on the facial nerve, or can result from a prior facial palsy.
A number of treatments are available for these problems, with the mainstay of therapy being the use of Botox, or botulinum toxin. We were one of the original investigators who worked with botulinum toxin when it was still experimental. We have experience in over 3,500 Botox treatments since 1984.

Loss of an Eye

The removal of an eye is an operation which may be made necessary either by severe trauma, or by tumors involving the eye itself or the eye socket. It is an operation we of course try to avoid if at all possible.
Fortunately, there have been many advances in the rehabilitation of the patient who has lost an eye. Dr. Gonnering has a special interest in this problem, and was asked to deliver the 1994 Ruedemann Lecture at the American Academy of Ophthalmology on this subject.
